Rei Suzuya was kidnapped at a young age and raised as the pet of a powerful Ghoul known as Big Madam. He was trained to perform circus acts for Ghoul customers, and earned "Good Boy Points", which was Big Madam's system which she used to torture him. Eventually, he began working in the Restaurant as a Scrapper named "Juuzou". When he killed someone for the first time, he was sick for several days but eventually his sense of morality faded. Big Madam, who he considered his mother, sometimes dressed him as a girl because she began to worry that he would become ugly when he grows up. To prevent this, she performed a crude castration with a hammer and forced him to live as a girl.
